Task description
For further information, including instructions for each part and specific due dates, please see the Blackboard Task Sheet for this assessment item. In this task, students are required to complete and submit a Clinical Practice Portfolio over the course of the semester in order to document clinical performance within the patient clinic. Students must complete the portfolio for ALL clinical, dental assisting, fail to attend, and observational sessions.
To achieve a pass, the following conditions must be met:
Course-specific session sign-off forms
TASK DESCRIPTION: These forms are essential for tracking performance and must be completed at the end of every AM or PM session where you see a patient.
Submitted by the student: Session records and case items
Description: Students are required to submit a record of the item codes and clinical notes completed for each patient for the session. If no patient was seen, the Dental Assisting/FTA box must be selected.
Submitted by the Clinical Educator: Session performance assessment
Description: Clinical Educators assess the student under their supervision for the timetabled clinical session. Note, multiple sessions may occur in one day. Student marking expectations for each semester are calibrated to your year and stage in the DMD program. You are not being marked against the performance of a fully qualified dentist, but rather according to where you should be for your level of clinical training. Clinical educators will adjust their marking expectations accordingly as you progress throughout the program.
Session mark:
Each section of the rubric will be awarded either:
• Expectations met; OR
• Expectations NOT met.
The rubric consists of the following categories:
• Fitness to practice items (MUST PASS)
• History taking, examination and clinical information gathering
• Critical thinking, diagnosis and treatment planning
• Professionalism, communication and response to feedback
• Record keeping
• Time management
• Technical execution of treatment
End-of-semester portfolio review and case tallies
This task helps to reflect on clinical progress, feedback integration, and future growth.
The component includes two parts:
1. Case tallies for the semester AND a de-identified Patient Management System Treatment Report uploaded, and
2. An End-of-semester Reflection and Feedback Form.
Minimum performance requirement: There is no minimum number of case requirements.
Assessment security
Secure assessment: This assessment is designed to protect academic integrity and assess your understanding of course concepts. You must only use the approved materials and tools for this assessment task. Use of prohibited materials in this task may be investigated and treated as academic misconduct. See assessment instructions for more details.
Hurdle requirements
This is a must-pass item.Submission guidelines
The submitted Logbook must be COMPLETE in all the following respects and submitted by the published due date, which will then be assessed as part of this assessment task. Any incomplete or missing information can result in a failing grade in this must-pass assessment task. For further information, please see the Blackboard Task Sheet for this assessment item.
- Students must ensure that their supervisors complete the Logbook for all clinical, observational, and assisting sessions. The electronic Logbook must be completed with regard to patient information (de-identified) and treatment codes/activities twice daily (AM and PM sessions), and must be signed off by a clinical supervisor. If no feedback (written or verbal) is provided by the supervisor during a session, the student must proactively seek feedback immediately after the session. The student should then record all feedback in the comments section and obtain the supervisor’s sign-off to confirm the discussion.
- A weekly structured reflection must be written around a specific case undertaken during the week, particularly where the supervisor/s have provided constructive feedback, or a grade of expectations not met was received.
- Treatment summary sheets ("tallies") must be submitted for all treatment items in week 19.
Deferral or extension
You may be able to apply for an extension.
The maximum extension allowed is 2 days. Extensions are given in multiples of 24 hours.
Late submission
You will receive a mark of 0 if this assessment is submitted late.
A late or non-submission will result in 100% penalty of the mark and therefore a fail for the assessment task.

Task description
Students are required to select a multidisciplinary case from patients managed in Semester 1 clinical work. The case selected should have elements of care planning/management from at least three distinct dental disciplines, such as Endodontics, Fixed/ Removable Prosthodontics, Oral Medicine, Oral Pathology, Orthodontics and Periodontics.
The information from the managed case should be collated into sections within a PowerPoint template provided on Blackboard. This template walks the student through all required information to be collected and presented, including patient history, any presenting complaint/s, the clinical examination, diagnostic workup, the care plan, including a discussion of rationale and decisions specific to the components and sequencing of the plan and the delivery of care.
This PowerPoint will be submitted online to the course coordinator via Blackboard one week prior to the oral presentation date.
Students will be assessed by a panel of two examiners on four aspects of their selected case:
- Clinical Information Gathering
- Critical thinking, Diagnosis and Management Planning
- Clinical Treatment and Evaluation
- Discussion and Reflection
Detailed information on task description, requirements for case selection, PowerPoint template and presentation details will be provided on the course Blackboard site in Semester 2.
This Oral Examination will be recorded and retained as per university policy. The recording will be stored in a secure manner and will only be accessed if required for the purposes of moderation of marking, provision of feedback to the student, and/or re-marking following a successful re-mark application.
