The aim of this course is to provide students with opportunities to explore the nature and lived experience of individuals who have chronic issues. A key tenet of this this course is the centrality of the individual and their support systems, including families, and the registered nurse's role in supporting these people. The course is situated in a primary healthcare framework and enables students to develop their theoretical knowledge and skills around health promotion, prevention, early intervention and the promotion of self-management in the context of chronic conditions. In addition, students will explore care planning, interprofessional teamwork, rehabilitative care and palliative care. The course content will be delivered over a 13 week period, utilising inquiry-based learning, online learning activities and self-directed learning.

Aims and outcomes

Apply and evaluate a population health approach to understanding chronic conditions to facilitate health promotion and prevention principles when assessing individuals.

Critically analyse and apply evidence-based approaches to supporting individuals with a chronic health issue and their significant others.

Critically analyse and evaluate differences between models of care in acute and non-acute healthcare environments to explore and evaluate the role of person-centred care in the context of chronic illness.

Critically analyse the role of the registered nurse and the interprofessional team in supporting individuals with chronic conditions as they transition into and out of various care settings.
LO6.
Critically analyse principles of habilitative, rehabilitative and palliative care for individuals with chronic conditions with a patient-centred focus.

Task description
Students will demonstrate their knowledge and understanding of course content by completing three (3) online quizzes (via Blackboard). Quizzes will be completed in Week 5 (Quiz 1), mid-semester break (Quiz 2), and the end-of-semester exam block (Quiz 3). The content of the quizzes will be drawn from the online modules (including readings) and IBL activities. The quizzes will be open from 0900 hours on Tuesday and close at 1300 hours on Wednesday. Students will have ONE ATTEMPT to complete each quiz.
This assessment item is eligible for an extension request on medical grounds and/or other exceptional circumstances. See Section 6.1 for extension policies and procedures.
AI can be used to assist with classwork and study; however, the use of AI is not permitted for any assessment item in this course.

- The extension granted should be in proportion to the period of illness or disruption caused by the exceptional circumstances and will typically be no more than the specified maximum extension length.
- Students on a Student Access Plan (SAP) with a clause on extension requests must still adhere to the extensions request policy outlined. Students need to attach the SAP and a note outlining their current situation with the online extension application. A maximum of 7 days will be granted on the basis of an SAP. An extension request beyond 7 days will require additional supporting information as this request will not be considered on the basis of the SAP, but rather additional events (whether they relate to the circumstances of the SAP or not).
Outcome of application
- Extension requests are generally processed within 2 working days. Students can see the outcome of their application by logging in to my.UQ.
Late submission
You will receive a mark of 0 if this assessment is submitted late.
The online quizzes will lock and cannot be commenced once the due date and time have passed. Quizzes will save and submit automatically once the due date and time have passed.

Task description
This assessment item requires you to work in small groups to develop and deliver a PowerPoint presentation that demonstrates your understanding of the needs of the person with chronic illness as they live in the community, and the role of the registered nurse as a coordinator of care. Students will work as a team to develop a program for people with chronic illness that builds their self-efficacy and ability to self-manage their illness. The task description and marking rubric for this assessment are available via the "Assessments" tab on the NURS7126 Blackboard site. AI can be used to assist with classwork and study; however, the use of AI is not permitted for any assessment item in this course.

One member of each group will upload a copy of their group’s PowerPoint presentation via the submission point on Blackboard by the due date. Please check the companion website assessment area for specific instructions on this process. A link will be made available in the assessment area of your Blackboard course to allow you to submit your presentation electronically.

Additional learning activity information
Group based learning is scheduled in your timetable as a tutorial. Students have the opportunity to meet fellow students and discuss and work on content items for the course. Each IBL session is presented to assist in meeting the learning aims and objectives of the course, including establishing collaborations for the group project. Students report finding IBL beneficial in networking and extending their learning within a small group environment supported by nursing academics in the school, and attendance at all IBLs is expected.
