You are required to create a case report that presents a vivid portrait of your work with a client, family, or treatment group in a mental health setting, ideally while utilising Cognitive-Behaviour Therapy (CBT).

Report Content:

  2. Formulation and Treatment Planning: Detail the formulation of the case and outline your treatment plan, highlighting the collaborative process involved in developing this plan.
  3. Process of Work: Explain the process of the work you have done together, including the CBT-informed assessment and treatment strategies used or considered for this clinical work.
  4. Outcome or Current State: Provide your understanding of the outcome or the current state of the work, reflecting on the progress and any remaining challenges.

Report Format:

  • This is a formal report and should be presented in a format appropriate to your work environment.
  • Demonstrate developing competence in assessment, formulation, collaborative treatment planning, and, as appropriate, CBT-informed psychotherapeutic interventions.
